Special Educational Needs and Disabilities (SEND) Services Feedback

Bexley's Local Area Special Educational Needs and Disability (SEND) Partnership is working to make improvements to the support and services provided for children and young people who have special educational needs and/or disabilities and their families.

We are interested to understand how it feels to be a child, or young person with SEND or their parent/carer in Bexley.

General experience

1. Family experience survey - This survey asks if you are aware of different support available, if practitioners give clear information and advice, and if your child gets the right support from a range of different services, people, and opportunities in their lives. This takes around ten minutes to complete.

EHCP Annual Review Feedback

2. Parents/Carers survey - This survey will be used to inform improvements to the Annual Reviews for EHCPs. The survey is eight questions long and should take around five minutes to complete. (If you have more than one child, fill in a separate form for each child.)

3. Children and Young People survey - This survey will be used to inform improvements to the Annual Reviews for EHCPs. The survey is eight questions long and should take around five minutes to complete.

Quality of EHCPs Feedback

4. Parents/Carers survey - This survey asks parents and carers ten questions to help us improve EHCPs. It may take around five minutes to complete.

5. Children/Young People survey - This survey asks the child four yes or no questions to help us improve EHCPs.

6. Special Educational Needs Co-ordinators (SENCOs) survey - This survey asks SENCOs five questions to help improve EHCPs. It asks if they accurately reflect the child's needs and are they easy to understand.
